Please follow the instructions  below to complete all athletic forms.  

Rank One Instructions

1. Follow this link or scan the QR code to open Rank One paperwork.

2. Click “Proceed to Forms” after reading the welcome note

3. If you have a login from last year, login to your account.  If you did not, you may create one under the “New to Rank One? Create New Account” bar or click “Continue as a guest”.  An account is not needed to fill out paperwork.

4. The next page will have all forms needed to be filled out.  They are “Medical History” “UIL Forms Signature Page” “Emergency Medical Consent” and “ Medication Permission”.  All forms will need to be signed by both parent and athlete.

5. Click on Medical History. At the top, fill out the student athlete’s first and last name (Must be their legal name that is in the school system) followed by their student ID (you can get this off of Skyward) and what school they will be attending for the 2021-22 school year.  Input all information in the form below that and both parent and athlete sign and check the “I agree” box at the bottom. Once finished, click submit and it will return you to the forms page.

6. Click on UIL Forms Signature Page.  At the top, fill out the student athlete’s first and last name, Student ID and what school they will be attending for the 2021-22 school year.  On this form, there will be 4 check boxes and 5 blue documents to be opened and read through.  Once you’ve read through the information, check the checkboxes next to the blue links.  Afterwards, fill out the rest of the information, and both parent and athlete must sign and check the “I agree” box at the bottom.  Once finished, click submit and it will return you to the forms page.

7. Click on Emergency Medical Consent.  At the top, fill out the student athlete’s first and last name, Student ID and what school they will be attending for the 2021-22 school year. Read through the information following, parents will input the student athlete’s name and sign the first signature box, click “I Agree” under the date and fill out the rest of the information down the rest of the page, then click submit.  You will return to the forms page.

8. Click on Medication Permission.  At the top, fill out the student athlete’s first and last name, Student ID and what school they will be attending for the 2021-22 school year.  Read through the information, parents will input the student athlete’s name, give or deny consent for medication, and sign in the signature box. Read through the information following talking about the option of purchasing supplemental insurance through the school and sign and date.  Click submit and it will return you to the forms page.

Once these forms have been filled out, they will be sent to Kam and Brandon to be reviewed and accepted.  Bring a paper copy of your physical to either one of the athletic trainers and you will be good to go!!
